This is a fully on‐site, patient‐facing role where RN case managers are assigned to specific units and work collaboratively within an interdisciplinary team of physicians, bedside RNs, social workers, and care coordination assistants, participating in or leading daily progression‐of‐care rounds, with no people management responsibilities. Duties include high risk screening, assessment, discharge planning, and coordination of post-acute care services.
